The Finding – Close Phase is where you report to the authority how you have implemented the corrective actions stated in the Act Phase. For example, if the finding was that there was no safety management manual, the implementation would be that you have now developed one and it is available. In this section, you explain to the authority how the implementation was completed, attach any supporting evidence, and then submit the finding for closure.

To access the Close Phase, click Close in the phase tracker located on the right side of the screen.

In the Implementation of Corrective Action Plan section, you enter details on how you implemented the actions stated in the Act Phase of the finding. To add your information, start typing in the text field, and a Save button will appear below. Once you have entered all the details, click the Save button to save the information.

In the Attach Evidence section, you upload documents or files that support how you implemented the corrective action plan. For example, if your corrective action plan stated that you would develop the Safety Management System manual, then in the implementation section you would confirm that the manual has been developed and attach the manual here as evidence.

In the Submission to Authority section, once you have entered the implementation details and attached evidence (as applicable), you will need to submit the finding for closure. Click the Submit button in this section to send the closure request. The inspector at the authority will be notified via email and will review the implementation details to determine whether they are acceptable for closure. If the implementation is acceptable, the inspector will close the finding. If it is not acceptable, the inspector will re-open the finding and provide a comment explaining the issue. You will be notified of both outcomes via email.

The Acceptance by Authority section is used by the inspector at the authority. If your implementation details and attachments are sufficient, the inspector will close the finding in this section.
