Findings – Capture Phase
The Capture Phase page displays all the information related to the finding assigned to you. This includes the detailed description of the finding, the items that failed, and any supporting evidence that highlights the issue identified during the audit. It clearly outlines the problem and what needs to be addressed to resolve the finding.
Please note that the Capture Phase is a read-only phase, meaning you cannot make any changes on this page.
Your role in this phase is to carefully review and take note of the issues presented.

The finding details section of a finding (within the capture phase) provides the following information about the finding:
- Finding Number – The unique identification number assigned to this finding by the authority’s inspector.
- Finding Title – A brief title summarising the issue identified in the finding.
- Audit Number – The unique identification number assigned to the audit related to this finding by the inspector.
- Risk Category – The risk classification assigned to the finding.
- Severity – The level of severity assigned to the finding.
- Audited Organisation – The name of the organisation that was audited in connection with this finding.
- Created By – The name of the inspector who created the finding.
- Date of Finding – The date on which the inspector recorded the finding.
- Inspector – The name of the inspector responsible for issuing the finding.
- Released By – The name of the inspector or other individual who released the finding to you or the organisation. The finding will only be visible once released.
- Released On – The date the finding was released to you or the organisation.
- Specific Finding – A detailed description of the finding and the issue identified during the audit.

The Finding Relationship (within the capture phase) section allows the inspector to link findings together. An inspector can link the current finding to a previous one issued if the issue has recurred or repeated, showing that the problem has occurred before. If there is a linked finding attached to the current one, click the Play button to view it.

The Applies to Approval Types section (within the Capture Phase) displays the specific approval type associated with the finding. For example, if the finding relates to Flight Operations, the OPS approval type will be selected. If it pertains to Dangerous Goods, the DG approval type will be selected. These approval types also determine who within the audited organisation will be notified or able to act on the finding. For instance, if the Dangerous Goods (DG) approval type is selected, users associated with Airworthiness will neither be notified nor have access to view the finding.
In the image below, only the OPS approval type is selected, which means only users assigned to this approval type will be notified and able to act on this finding.

The Failed Items section (within the Capture Phase) displays the specific items that have been failed for this finding. A finding may include multiple failed items.
Each failed item may also have an associated regulation reference to indicate the relevant regulation. To view the regulation reference, click the i icon next to it.
In the image below, the inspector marked No for the assessment of the item, Does the operator have a Safety Management Manual in place?, and provided comments detailing the issue identified.

The Other Failed Regulations section (within the Capture Phase) displays any additional regulations the inspector may wish to include. For example, this could cover health regulations or other regulations not directly related to aviation.

The Categories section (within the Capture Phase) displays how the inspector has categorised this particular finding. For example, in the image below, since the finding relates to the absence of a Safety Management Manual, the inspector selected the category Manuals, with the supporting evidence being that the documentation or records are unavailable. The inspector uses this information as part of their heatmap to highlight the issues that need to be addressed.

The Ownership and Deadlines section (within the Capture Phase), shows the following information:
Owner: This field shows the name of the organisation or individual who owns the finding. Below this, a list displays the names of people within your organisation who have been allocated the finding. In the image below, only John Smith has been allocated, meaning he is the sole person notified and authorised to view and act on it.
Action Plan Due By: This field indicates the due date set by the inspector for submitting your action plan to the authority.
Closure Due By: This field shows the due date set by the inspector by which the finding should be submitted for closure.
Allocated To: This field is used when an inspector assigns a specific person from the finding distribution list to be responsible for the finding.
Escalated To: This field is used when an inspector escalates the finding to a specific person within the finding distribution list.

The Attachments section (within the Capture Phase) includes any evidence the inspector gathered during the audit. For example, if the inspector found a dent on an aircraft wing, a photo of the damage would be attached as evidence. To view an attachment, click on its title, or click Download to save the file to your device.

You can download the finding as a PDF or Excel file (including associated attachments, if selected) by hovering over the Download button in the lower right corner and selecting the appropriate option.

If you believe you will be unable to submit your action plan or close the finding by the deadline set by the inspector, you may request an extension.
In the Ownership and Deadlines section, the Action Plan Due By and Closure Due By fields each have a Request to Postpone button.
Clicking the button will open a pop-up where you must enter the reason for the postponement and the new deadline you wish to request. After completing these details, click Request Postponement. The inspector at the authority will be notified of your request and will either approve or reject it.
- If approved, you will receive an email notification, and the due date will be updated to the new requested date.
- If rejected, you will also be notified by email, the original due date will remain, and the inspector will provide a reason for the rejection.
