Findings – Act Phase
The Finding – Act (Action) Phase is where you begin addressing the finding and submit your plan to the authority detailing how you intend to resolve the issue.
In this phase, you can make changes and enter your corrective action plan. To access the Act phase, click Act in the phase tracker located on the right side of the screen.

The first field in the Act phase is the Immediate Corrective Actions field, where you enter your immediate corrective actions for the finding. Immediate corrective actions are typically required to quickly mitigate safety hazards identified during audits or inspections, before a more comprehensive corrective action plan is developed and implemented. If you have nothing to enter or the field is not applicable, you may leave it empty or enter “N/A”.
To enter your immediate corrective actions, click the Edit button at the bottom left of the finding. After entering the information, click Save.
Tip: With the update to Centrik Version 5, you can now enter edit mode by simply clicking on the field you wish to modify. However, remember to click Save once you have completed your changes.

The second field in the Act Phase is the Root Cause Analysis field, where you enter your analysis of the underlying cause or causes that led to the finding. Root Cause Analysis identifies the fundamental reason the issue occurred, helping to ensure effective corrective actions can be put in place to prevent recurrence. If you have nothing to enter or the field is not applicable, you may leave it empty or enter “N/A”.
To enter your root cause analysis, click the Edit button at the bottom left of the finding. After entering the information, click Save.

The third field in the Act Phase is the Root Cause Correction field, where you enter the actions you will take to address and eliminate the underlying cause or causes that led to the finding. Root Cause Correction focuses on implementing solutions that directly tackle the identified root cause, helping to prevent the issue from recurring. If you have nothing to enter or the field is not applicable, you may leave it empty or enter “N/A”.
To enter your root cause correction, click the Edit button at the bottom left of the finding. After entering the information, click Save.

The fourth field in the Act Phase is the Corrective Action Plan field, where you enter a detailed plan outlining the steps you will take to resolve the finding. The Corrective Action Plan should address both the immediate issue and any underlying causes, ensuring that the problem is fully rectified and prevented from recurring. If you have nothing to enter or the field is not applicable, you may leave it empty or enter “N/A”.
To enter your corrective action plan, click the Edit button at the bottom left of the finding. After entering the information, click Save.

The Deadlines section (within the Act Phase), shows the following information:
- Action Plan Due By: This field indicates the due date set by the inspector for submitting your action plan to the authority.
- Closure Due By: This field shows the due date set by the inspector by which the finding should be submitted for closure.
Please note that the Action Plan Due By and Closure Due By dates in the Capture Phase and Act Phase are linked.

The Submission to Authority section (within the Act Phase) is where you submit your corrective action plan to the authority once all required information has been completed and entered.
To submit your action plan, click the Submit Action Plan button. The inspector responsible for the finding will be notified by email and will review your submission when possible. If the plan is acceptable, it will be approved; if not, it will be rejected.
- If accepted: You will receive an email notification confirming the approval, and the finding will become read-only.
- If rejected: You will receive an email notification stating the rejection, along with a brief explanation of the reason. You will then need to amend your existing plan with the necessary changes or additional information and resubmit it for review and approval.

The Acceptance by Authority section (within the Act Phase) is used by the authority’s inspector to either accept or reject the corrective action plan. If the plan is accepted, you will be able to see the name of the person who approved it and the date it was accepted.

The Attachments section (within the Act Phase) is where you upload any documents or evidence to support your corrective action plan. You can add files by clicking Add Attachment or by dragging and dropping them into the section.

The Add Comment section (within the Act Phase) allows you or the inspector to add comments to request clarification, ask questions, or respond to a specific issue raised. To add a comment, click the Add Comment button, enter your message, and then save it.
The person who posted the comment will receive an email notification whenever a new comment is added.

You can download the finding as a PDF or Excel file (including associated attachments, if selected) by hovering over the Download button in the lower right corner and selecting the appropriate option.

If you believe you will be unable to submit your action plan or close the finding by the deadline set by the inspector, you may request an extension.
In the Ownership and Deadlines section, the Action Plan Due By and Closure Due By fields each have a Request to Postpone button.
Clicking the button will open a pop-up where you must enter the reason for the postponement and the new deadline you wish to request. After completing these details, click Request Postponement. The inspector at the authority will be notified of your request and will either approve or reject it.
- If approved, you will receive an email notification, and the due date will be updated to the new requested date.
- If rejected, you will also be notified by email, the original due date will remain, and the inspector will provide a reason for the rejection.
Please note that the Request to Postpone buttons for the Action Plan Due By and Closure Due By dates in the Capture Phase and Act Phase are linked.
This means you can request a postponement from either the Capture Phase or the Act Phase.
